Whether you are a new patient or have been coming to our office for years, we sometimes ask for updated forms for your account.
As of November 1, 2017, Dr. Doyle and his staff have started the process of a chartless practice. This means we are doing some updates on our policies and procedures. Beginning November 1st, we will be asking every patient for an updated medical & dental history form. We will not ask you to fill out a new form every visit, but rather every 3 years.
However, we will be verbally asking if there are any medical updates at each appointment. This includes medications, hospital visits, pediatrician changes, allergies, etc. Please make sure to inform us of these changes since your last visit.
Also, we have updated our HIPAA form. We are asking for 1 form to be completed per household.
To save time, feel free to print these forms at home and complete them at your convenience. You may bring the updated forms with you to your next scheduled appointment or login to your patient portal and upload each form on the 'MY FILES' tab.

Patient portal is a great way to connect with our staff, upload forms and documents as needed, see your account and much more!
It's very easy! All you do is login to your existing account and from there you can access all of your information. Everything is safte and secure!
We are happy to announce you can pay your balance online!
CLICK HERE to login to your patient portal and go to the 'BILL & PAYMENTS' tab on the left side bar. From there you can easily see account balances for each patient and apply your payment. Once your payment has been processed and accepted you will receive an email notification.
